The question

To ask the Secretary of State for Health and Social Care, what proportion of patients at (a) Upton Hospital and (b) Wexham Park Hospital have been discharged into social care in each of the last 12 months.

The Department does not hold information on the proportion of patients that have been discharged into social care in individual hospitals in each of the last 12 months. NHS England publishes data detailing how many patients are discharged through each discharge pathway, broken down by trust. This includes data on discharge pathway 3, which involves discharge to a care home placement.
